You can complete your forms online using the Client Portal link above or print them out and bring them in.
If you chose to print them out, please download and complete the forms below before you come for your first appointment. To download a form, just hover your cursor over the page you wish to print. When you do, a footer appears at the bottom of the page. Click the icon that looks like a page with an arrow on it and your form will open in another tab. You can print from this new tab.
Please note that there are two consent forms, one for children and one for adults. Also note that the Registration Information form is an editable .pdf. This means that you can type directly into the form and then print it off when you complete it.
